DENTAL INSURANCE

Federal Employees Dental and Vision Insurance Program (FEDVIP) provides supplemental dental insurance coverage. There is no government contribution to the premiums for this coverage; however, premiums are paid on a pre-tax basis. Most Federal employees are eligible to enroll.

You have 60 days from the date of your appointment to elect to participate in the plan. You will make your election for dental coverage directly with BENEFEDS. You can contact a BENEFEDS Customer Service Representative by calling 1-877-888-3337 , 8 a.m. to 7 p.m., Monday through Friday, Eastern Time; or by email at info@ltcpartners.com. The TTY number for the hearing impaired is 1-877-889-5680. You can access information and resources regarding the FEDVIP program online at www.benefeds.com. You cannot enroll in a FEDVIP plan using EBIS.

Dental enrollment includes the following options:

  • Self (covering only the employee)
  • Self Plus One (covering the employee plus one eligible family member)
  • Self and Family (covering the employee and all eligible family members

Premiums will vary by plan and by enrollment type (Self, Self Plus One, or Self and Family). These plans have premiums determined based on where the enrollee lives, called a rating area. Each dental plan may have up to six rating areas, including an international rate.

Dental coverage includes the following options:

  • Class A (Basic) services, which include oral examination, prophylaxis, diagnostic evaluations, sealants and x-rays.
  • Class B (Intermediate) services, which include restorative procedures such as fillings, prefabricated stainless steel crowns, periodontal scaling, tooth extractions, and denture adjustments.
  • Class C (Major) services, which include endodontic services such as root canals, periodontal services such as ingivectomy, major restorative services such as crowns, oral surgery, bridges and prosthodontic services such as complete dentures.
  • Class D (Orthodontic) services with up to a 24-month waiting period for eligible dependents up to age 19.
